An issue report is a formal document that outlines changes or problems in a project. This document is important for ensuring that all stakeholders are aware of any potential impacts to the project.

An Issue Report is a formal document that outlines changes or problems in a project. This document is important for ensuring that all stakeholders are aware of any potential impacts to the project.

Issue reports are generally only written for significant requests for change, moving to “off-specification”, or large problems orconcerns.

So what type of issues make it into the issue report?

Issues that may require an Issue Report include

1. any change of significant impact (such as cost, schedule, quality)

2. deviations from the work plan or specification without agreed trade-offs

3. issues with equipment or facilities beyond those expected in the project delivery arrangements

The Issue Report should be a short document, typically no more than 1 to 2 pages long. It should contain an impact assessment and recommendations for change, as well as an overall description of the issue.
